Checklist for EMT-Basic Certificate
All checked items listed below are required for a complete application. If items are missing, your application may not be processed.
- 18 years or older by program completion date
- High school diploma or GED from an accredited school
- Completed application (click here for Texas Common Application).
Submit the following materials and documents to the Registrar at the address above, before the application deadline:
- $45 application fee – On-line payment, or check or money order sent to the Registrar at the address above. Write your application number on the check or money order to assure proper processing. Your application cannot be processed without payment being received by the deadline date.
- Official high school transcripts, GED, or college/university transcripts for every institution you have attended, or – If official transcripts are sent to you, you may send them to the Registrar if the seal on the envelope is not broken.
- TOEFL score (Test of English as a Foreign Language) – Required only of international applicants from countries where English is not the native language; minimum score of 68 (Internet), 220 (computer), or 560 (paper) is required.
- International Transcript Evaluation (international students, only) – Transcripts of coursework taken at colleges/universities outside the United States must be submitted to the Registrar, along with a course-by-course evaluation by an approved credentialing organization. - 30 semester credit hours (minimum) of coursework in EMT-Basic through EMT-Paramedic courses
- Current EMT-Paramedic certification, state or national

- Cumulative grade point average (GPA) of 2.0 for9 all college courses
- Completed application (click here for Texas Common Application).
