Checklist for the Master of Occupational Therapy Program
- All checked items listed below are required for a complete application. If items are missing, your application may not be processed.
- All prerequisite courses completed by the end of spring semester of the year you wish to enter the program, with a minimum grade point average (GPA) of 3.0 calculated for prerequisite courses. Click here for prerequisites.
- Complete at least 20 hours volunteer and/or observation under the general supervision of a registered occupational therapist (see Documentation of Experience below).
- Completed application (click here for Texas Common Application).
Submit the following materials and documents to the Registrar at the address above, before the application deadline:
- $45 application fee – On-line payment, or check or money order sent to the Registrar at the address above. Write your application number on the check or money order to assure proper processing. Your application cannot be processed without payment being received by the deadline date.
- Official transcripts from every college or university you have attended – If official transcripts are sent to you, you may send them to the Registrar if the seal on the envelope is not broken.
- Two (2) reference forms – It is not necessary to mail the completed forms in separate, sealed envelopes. Click here to download or print reference forms.
- Documentation of Experience – Document at least 20 hours of volunteer and/or observations experience. Click here to download or print the form.
- Request of State Required Prerequisites (if required) – Necessary if you already have a bachelor’s degree but are lacking any UTHSCSA Core Curriculum requirements. If you waive any core curriculum requirements, you will also waive the Bachelor of Science in Health Care Sciences (BSHCS) that is awarded after one year in the Occupational Therapy program. - Request for Waiver of Program Prerequisites (if required) – Necessary if you wish to request substitution of a program prerequisite course a waiver of program prerequisites. Include a copy of the course number and title, description, college/university where it was taken, and state which prerequisite that it will substitute for. - TOEFL score (Test of English as a Foreign Language) – Required only of international applicants from countries where English is not the native language; minimum score of 68 (Internet), 220 (computer), or 560 (paper) is required.
